actually all that rpmbuild is doing is invoking patch(1).

The usual convention is that the suffixed file
(the bog standard default suffix in patch(1) is ".orig")
is the BEFORE while the unsuffixed file is the AFTER applying
a patch.

diff the two files to see what's changed where.

and "man 1 patch" should give you more than you ever
wanted to know about patching.
